Blame, excuses, explanations, contextualisation ... They get dull after a while.
Here're some ways I can take responsibility:
"I was disorganised."
"I did not prioritise it."
"I did not schedule it."
"I did not monitor my diary."
"I do not have a system for making sure I get things done."
"I took on too much."
"I lied."
Etc.
Suddenly the air comes back in the room and everyone can breathe.
